every full-time undergraduate and graduate student must also have a completed Student Health and Immunization Record on file in the Student Health Center.  Please note the following important requirements:

To comply with New Jersey legal requirements, and Stevens’ requirements, you must be immunized with two (2) doses of measles, two (2) doses of mumps, and two (2) doses of rubella vaccine prior to attending any classes.  This vaccination is often called an “MMR”.

The law also requires that all new students residing in Stevens-owned or leased housing receive one dose of meningitis vaccine. The vaccine must be quadrivalent in nature, targeting serogroups A, C, W-135 and Y: Menomune, Menactra, or Menveo vaccine is acceptable in the United States.  College health centers in New Jersey must provide information about meningitis disease and keep records of an enrolling student’s decision about whether or not to receive a meningitis vaccination if they are not residing in campus housing.  Please provide this information on the Student Health and Immunization Record.

Stevens Institute of Technology also requires students to have a PPD/tuberculin skin test within six months of attending classes.  This test is to be administered REGARDLESS of whether or not you received BCG vaccine as a child.  A recent chest x-ray written report is acceptable as well (please do not bring us the actual x-rays).

Proof of the Hepatitis B series of vaccinations is also required of all new incoming full-time undergraduate and graduate students. All international graduate students on F-1 or J-1 visas sponsored by Stevens are considered full-time students. The first two vaccinations should be one month apart and the third vaccination should be at least six months after the first. We also accept proof of immunity if you have a copy of a blood titer test, HbsAb, to verify immunity. The Hepatitis B Surface Antibody must be printed on the report as either “+” or “positive”.

Stevens also requires documented proof of two Varicella (Chicken Pox) vaccinations or the date that the disease was confirmed (usually in childhood).

It continues to be an important aspect of New Jersey Immunization Law to make sure that patient names, types of vaccinations, and results are translated into English on immunization documents before they are presented for approval by the Student Health Center.  You can download and print the Student Health and Immunization Record by clicking here.  Note that the form(s) must be completed in full, signed, and dated by a healthcare provider/MD. Please fill out this Student Health and Immunization Record so that you can satisfy New Jersey Higher Education law and Stevens’ requirements. An incomplete Student Health and Immunization Record will result in a Registrar’s Hold being placed on your account. This means you will not be able to register in person or via the Web for any future classes until all forms are received by the Student Health Center.
