all students born on or after1/1/57need to provide proof or two measles and one German Measlesshowing a positive titer must be received in our office prior to registration. Stuedents born prior to 1/1/57 need to complete only the front of this form and Section B on the back. Section A this box is to be completed by medical or authorized personnel only immunizations in order to be considered official, this section or any additional records forwarded to us must meet tow out of the four following requirements:signature of authorizing person, license number of authorizing person, letterhead, office stamp with complete address. all records must also include the student's name and front cover of all documents. any changeink/handwriting or use of white-out must be completely reauthorized. we reserve the right to interpret the validity of all documents submitted
