Immunizations are generally not required of IUPUI students unless they will be living in on-campus housing. Some graduate departments, especially in the medical field, require students to show proof of immunization. If you are a graduate student, please check with your department to see if there are any requirements.
If you will be living in on-campus housing, you are required to verify that you have had a MMR (Measles – Mumps – Rubella) immunization. You will need to complete the “Residential Student Immunization Requirements” form available at http://health.iupui.edu/st_he_form.html .
All students are also required to complete the Immunization Compliance Form through Onestart. The purpose of this form is for students to verify that they are aware of the risks of meningitis and the benefits of immunization. An actual immunization is not required. More information is available at http://registrar.iupui.edu/registration-guide/meningitis.htm. Students not completing this form will not be able to register for a second semester until the form is completed.
