The following is my material list, hope XDJM here can check it to see if it is complete or right? Please give your precious suggestions. Thank you in advance. (BTW: which name should I write in the envolope INS or BCIS?)

You need add covered letter as well. You can find more information in here: zhi4ding3 (in chinese)

To Lilycool: What is zhi4ding3? Can you explain it.
To Acezh: 邮寄复印件，作去Moxico二手准备? I'm not prepare for going Mexico and from the others XDJM Tiezi mailing copy of I-94 is the usually thing since it needn't to mail the original I-94 form. I don't know if I am right or not, hope you and other XDJM give some advices. Thank you very much.
