[干货]UIUC行前准备之体检疫苗和保险

经过漫长录取季，获得录取通知的一瞬无疑是如释重负。然而接下来的一系列问题却也十分棘手——签证申请，体检，行前准备，租房购车等等。去年这一堆杂事折腾得我头昏脑胀，却也整理出一系列干货。

【体检和疫苗】

•        只要确定了去哪个校区就可以开始准备打疫苗，所有疫苗需要在到校前完成。
•        有些疫苗不止有一针，还会有加强针，而且某些疫苗不能同时打（例如，MR 和精白破不能同时打，中间要间隔 2 周 以上），所以要提醒学生尽量早点打，以防之后不能打完所有要求的疫苗（当然也可以在美国打，但是会比较贵）
•        UIUC 疫苗要求： http://www.uis.edu/healthservices/immunizations/facts/
•        基本包括Adult Tetanus（破伤风）, Diphtheria（白喉） and Pertussis（百日咳） / TD (破伤风白喉)(within the last 10 years), measles (麻疹), mumps (流行性腮腺炎), rubella(风疹)，polio (小儿麻痹症)， Meningitis Vaccine （脑膜炎）(all students under 22 years of age)
•        具体打疫苗的种类见‘IMMUNIZATION FORM’
•        其他：到校注册时需要打肺结核疫苗，但需要提交以上免疫表才能打
学校的说明：
Illinois state law requires all new students to provide proof of immunity to certain contagious diseases (among them: diphtheria, measles, mumps, rubella, and tetanus) before enrollment. All admitted students will receive an Immunization Information Form, which must be completed and submitted upon arrival on campus. All international students will be screened for tuberculosis at the time of enrollment in accordance with the Illinois Department of Public Health. All students please note: if the required Immunization Information Form is not submitted, you will not be eligible to register for future terms. Immunization Information Forms are also available online from the McKinley Health Center.

【Q&A】

•        小时候的疫苗本有用吗？
有用，把小时候的疫苗本找出来，可以去到街道医院，请他们帮你在表格上签字盖章。如果有些疫苗之前也 没有打过，可以进行补打。
•        去哪里打疫苗？
当地出入境检疫局
•        体检材料
1)        本人身份证或护照原件+复印件 1 份
2)        近六个月内拍摄的二寸免冠彩色照片 4  张 –
3)        UIUC预防接种表格
4)        打印《外来医学表格受理须知》（这个如果忘记，工作人员会现场给）
5)        本人既往原始的预防接种记录原件+复印件 1份。
•        体检和疫苗完成后，会获得所谓的小红本和小黄本 。

【保险】

•        不需要自己买保险，学校会自动买好，账单会出现在 fall session 的 bill中，只管交钱就行。
