填1294e表遇到问题，请问一下

填1294E表遇到几个问题，请问大家一下：

1： national identity document：
NATIONAL IDENTITY DOCUMENT
Do you have a national identity document?t是指身份证上的信息吗？

2：在passport这一大栏里有两个小问：
    5 For this trip, will you use a passport issued by the Ministry of Foreign Affairs in Taiwan that includes your personal identification number? No Yes
* For this trip, will you use a National Israeli passport? No Yes
在电脑里在NO里打不了勾，这个大家遇到吗？


3:最后在签名的地方，可以直接在电脑上签中文名字吗？还是一定要将这个表全部打印出来用手签呀？
