请教体检表格的填写

递交完PVA的申请，收到DIMIA的邮件确认，同时收到体检的电子表格，看了一下，有些地方有疑问，向大家请教——先谢各位了~

1)根据‘Instructions For Offshore Medical Examinations’的说明，我是不是要填Form 160 和Form 26中的‘Office use only ’的以下内容：
‘File number ’(即file reference)？
‘visa class’(即157)??
ICSE Request ID是Permission Request ID吗？说明上说要填，可没有对应的空格啊？究竟填哪？

2) Form 160 中的
“6 How long do you intend staying in Australia?”
应选temporarily吧？2月开学12月应该能结束考试了吧？（我是一年的课程）填11个月？

3）Form 160中的
“7 Have you lodged an application at an office of the Department of immigration and Multicultural and Indigenous Affairs?
No   ->  At which office do you intend to lodge an application？
Yes   ->Which office?                                ”
应选yes? Which office 我是真没头绪。。。

office only 的千万不要填，是医院填的。
体检表没什么的，就把你会填的填了。不会的到医院去问问人家再填。

这个怎么选？填什么？
