The name you entered on your registration form MUST match the name on your photo identification. If you do not provide acceptable photo identification, you will not be allowed to test and your test fee(s) may be forfeited.
In accordance with the photo ID requirements, all identification must match the name on the letter of authorization or on the admission ticket. A marriage license will NOT be accepted. Please refer to the Registration Bulletin for information about acceptable Photo Identification.
Please be advised that your August 22, 2005 Subject Test payment of $150.00 was declined by your credit card company. Your outstanding balance of US$150.00 is currently due. An invoice statement explaining this matter was mailed to you on August 26, 2006.
