关于申请表的填写2

本人在填写The University of Queensland的申请表时遇到以下问题不懂,望各位指导一下,谢谢!

请问以上这项是必须填写的吗?若是必须要填的,那前面4个空怎么写啊?

好像是代理人的信息，你自己申请的话就没有代理人，应该不用填吧
