1. Quite the same. PhD or Master degree in science.
2. General at least. Some program ask additional Sub, depends on the univeristy and program. But GRE general at least.
3. Not familiar with your school, but I think it sounds like a combined program as transfer student to some other university to get two degrees, maybe Emory? Actually most other medical shools offers MD + PhD to 8 years program.
4. No need to take TSE, the USMLE step2 CS contains communication score, and yeas, it is very hard to pass that, needs lots of practice. One friend of mine has worked in US for 4 years, and failed the CS communication part. If doing PhD in US, it will be very busy, not much time for test preparation. But, for step 1 and step 2 CK, at least, no need to go to hospital, just all kinds of knowledge questions. If with some time to do observer in hospitals, step 2 CS and step 3 will not be too hard to pass. However, our goal is 99/99/pass CS first time/ 90+, right, it sure will be not easy, but possible.
5. You will need to come to US for Step 2 CS and Step 3. If you come out applying your self, it will be very hard to come back to China as physcian. Most times, people do post-doc and then post-doc, and post-doc again, until find some faculty position....... That's the regular career pathway for PhD students. But if you get excellent USMLE scores, great English, and better a green card during Pot-Doc, you will have some chance to get matched into residency program, to do 3-4 years of residency training, and then become a physcian at US. If not as physcian nor faculty, it will be very hard to go back China with a good job, when already old. But if you are a physcian or faculty and a green card in hand, mostly you will still stay working at US. So, over all, maybe you will not consider that much about working back home, especially after a having a baby in US......
