1) yes, in CV or if possible, in transcript. but honestly U.S schools only recognize a handful of top universities ( probably less than 10)... so it really doesn't matter that much...

even you wrote the rl and ur prof signed it... the rl are still "rl written by professors..." therefore u should write it the way a professor would write it. Of course if someone is writing a letter to others that person will introduce him/herself at the beginning of letter...

the schools that you apply for may or may not contact your professor... no one knows
