International doctors are those doctors who, regardless of where they may have obtained their primary qualification, do not have right of indefinite residency or are not settled in the United Kingdom (as determined by immigration and nationality law) or who do not benefit from European Community rights. Such a doctor is one who is not a national of one of the Member States of the European Economic Area, including the United Kingdom. This definition also includes those doctors who may have received a primary medical qualification from a United Kingdom University but are not settled nor do they have right of indefinite residence.

Doctors cannot carry out any clinical training whatsoever involving the management of patients in the UK unless they are registered with the General Medical Council (GMC).

The GMC, by granting registration, gives doctors the legal status necessary to undertake clinical professional duties. Enquiries about registration should be made to the GMC at least 9 months in advance of coming to the UK. Delay will occur if forms are not filled in correctly or if the current documents are not provided. The documents provided must be originals. Currently there are three kinds of registration with the GMC for those wishing to undertake postgraduate medical training:

Full
Provisional
Limited
The fact that a doctor has gained the right of entry to the UK does not override the legal requirement that appropriate registration with the GMC must be held.

Doctors are responsible for ensuring that they have appropriate registration: the Deanery is not able to negotiate with the GMC on behalf of doctors.

Limited Registration

Most international doctors will be granted Limited Registration when taking up their first post in the United Kingdom. This registration may be granted for employment supervised by a fully registered medical practitioner in training posts in hospitals or other institutes which have been approved by a Royal College or Faculty and the Postgraduate Dean. It may be granted for a particular post or grade, and/or specialty specified by the GMC. It is limited to a total period of five years.

After holding Limited Registration for a period of time (normally at least two years) doctors may apply for Full Registration. Further information can be obtained directly from the GMC.

PLAB ( The Professional and Linguistic Assessments Board) Test

Doctors seeking Limited Registration must first pass, or be exempt from, the PLAB test conducted on behalf of the GMC. To take this test of professional medical knowledge and competence, potential candidates must achieve adequate scores in a test of proficiency in English.

IELTS - International English Language Testing System

This is an English Language testing service recognised by the GMC. All international doctors are required by the GMC to take this language test even if their language of instruction at medical school was English. The GMC specifies the scores to be achieved, as do some Royal Colleges/Faculties as an entry requirement to their Overseas Doctors Training Scheme (ODTS).

Part 1 of the PLAB test and the IELTS may be taken at certain centres outside the UK. Further details can be obtained from the GMC.

Permit-Free Training (PFT)

'Permit-free training' is a special immigration status, which allows doctors and dentists to hold posts for postgraduate training in the hospital, and/or community health services without holding a work permit.

To qualify for PFT in the UK an overseas doctor must satisfy the Immigration Office upon arrival in the UK that:

1. He/she intents to undergo postgraduate training in a hospital.
2. He/she is currently registered or is eligible to apply for registration with the GMC (having already been exempted from the PLAB test).
3. He/she intends to leave the UK after their training.

The length of PFT relates to the training programme of the individual doctor. A training post must have Royal College educational approval and Postgraduate Dean's approval. The amount of time the trainee may be admitted to the UK to train in medicine is directly linked to the type of training being followed. Training falls into three basic types:

Extensions of permission to remain in the UK on a permit free visa, within the specified limits, require the support of the Postgraduate Dean, Training Programme Director and Chair of the Specialty Education Committee who must be satisfied that the trainee is making satisfactory progress in training. The Postgraduate Dean only deals with extension to current PFT visas. If the trainee has never had a permit free training visa his/her employer should apply on their behalf to the Home Office. The trainee should allow at least 8 weeks prior to the expiry of the current permit free training period.

If the trainee has been appointed to a post in another deanery with a start date soon after the end of their current permit free period date, the present deanery will sign the form requesting an extension provided the trainee supplies written evidence of the new post from the Medical Staffing Department.

Permit-free training arrangements have now been extended to cover doctors working in General Practice. This means that doctors with permit-free training no longer need to have a TWES (Training/Work Experience Scheme) certificate in order to receive funding for the GP Registrar component of their training.

International undergraduates are issued with a student visa - linked to each University Medical School. It is a 5-6 year programme leading to Bachelor of Medicine or Bachelor of Surgery. Admission is through the Universities of Colleges Admissions Services for the UK (UCAS).

The Home Office forms (PF/PGD and FLR(O) can also be downloaded from the Internet. The website is www.homeoffice.gov.uk.

If the doctor is coming to the UK to take a non-training position, then a work permit will be required which is recommended by the hospital or other person wishing to employ the doctor.

The International Doctors' Training Scheme

The College's Double Sponsorship Scheme has had to be temporarily suspended due to the inability to obtain supernumerary posts for overseas doctors entering the scheme. The UK Postgraduate Deans have intimated that they will not recognise, and therefore pay for, any such supernumerary posts. This applies to the majority of UK Deans. Representations made by the British Council, in conjunction with the National Health Service Executive have, so far, been ineffective. Therefore for practical purposes the College can no longer fulfil its commitment to overseas doctors wishing to come to UK via the College Scheme. (Royal College of Physicians Newsletter June 2002))
